Overview:

Case Management for Seniors in Using Action Learning Approach is a course which aims to equip individuals with the fundamental knowledge and skills in using Action Learning Approach when applying to geriatric case management process. The course also helps participants to apply system thinking as well as use problem –solving process for managing seniors with challenging behaviours.

Target Participants:

Social work professionals with at least 2 years in case management experience within the eldercare setting. This may include includes a broad range of social service practitioners, case managers, social workers, social service practitioners who are working in the eldercare setting.

Learners are assumed to possess an English Language (speaking, reading, writing and listening) proficiency level equivalent to GCE ‘O’ level or Employability Skills System Workplace Literacy (WPL) Level 6.

Learning Outcome:

By the end of the course, participants will be able to:

1. Apply a geriatric case management process in an eldercare facility setting

2. Use Action Learning (AL) in case management for eldercare

3. Apply systems thinking to promote holistic eldercare services

4. Manage challenging behaviours from seniors in an eldercare facility setting

Remarks:

This course includes assessment components.

Assessment may be conducted during and/or after the course. The grading and processing of assessment results can only take place after course completion. Learners who pass the assessment(s) will be awarded with an electronic Certificate of Achievement from SSI. You will receive an email notification when your e-Cert is ready for download in your NCSS e-Services account.

Trainer Profile:

Ms Lim Hwee Er, holds a Masters of Health Service Management (Univeristy of Sydney) and a Bachelor of Health Sciences (Hons) (OT) (University of Teesside, UK). As an occupational therapist, her 14 years of clinical experience working with the elderly people of various geriatric/psychogeriatric conditions spans from hospitals to community services including nursing homes, day centers and home-based services. She also has vast experience providing consultancy services in planning and implementing specialised services and programs such as early dementia programs and hostel. As an ACTA certified trainer, Ms Lim is involved in development and implementation of WSQ certified and regular training and workshops for professionals, family caregivers and the public in collaboration with various organizations such as Tsao Foundation, HPB, SSI and CSC on issues related to eldercare and well elderly programs. Ms Lim is currently in private practice with Goshen Rehabilitation & Consultancy Services.
