About The Role

Reporting to the HOD of Communities of Care (COC) the person is to support the roll out of Communities of Care model, Health District programmes and any other programmes related to COC.

Responsibilities
• Projects and Programmes
Lead or support the creation, planning and implementation of programmes and activities of COC and Health District programmes.

• Community Profiling
Perform outreach and community profiling to identify the needs and interests of the seniors within service boundary, recruit seniors to join COC and AAC programmes.

• Workflow with Partners
Engage with relevant stakeholders (Community care service providers, community partners and government agencies) to develop, pilot, communicate and review the workflows, protocols and processes that facilitate the integration of social and health care for the residents.

• Reporting
Assist the Project Lead with project and programme reporting requirements and in maintaining of the database.

• Asset Inventory
Identify, stocktaking, assess the strengths and limitations of community assets, including other Community Care service providers.


Candidate Requirements
Technical Competencies:

• Bachelor’s degree or Diploma holder with relevant experience in working with older persons.
• With 1 to 2 years’ experience in project management, business development or relationship management.
• Experience in developing and maintaining corporate partnership with networking partners.

Core Competencies:

• Demonstrates ability to conduct needs assessment for service users at the community.
• Able to plan, implement and evaluate programmes for the community.
• Good documentation and report writing
• Able to adhere to cost control and project scheduling of the projects and programmes.

Preferred achievements / characteristics:

• Dynamic, quick-thinking, adaptable and responsive to change
• Proven record in initiate or champion a project from conception to implementation
• Enthusiastic, self-motivated and enjoys working with seniors
• Good communication and interpersonal skills
