We are looking for passionate individuals committed to the cause of reunifying children and young persons who are in out-of-home care back to their families in a safe, timely and sustainable manner. - Provide intensive, home-based intervention and support to CYPs and families.- Conduct needs and risk assessment in accordance to the service framework.- Develop appropriate reunification case plans that is inclusive of the CYP and family’s opinions.- Implement and follow up on case plans and interventions.- Work with community stakeholders to address systemic barriers to reunification.- Provide regular updates and reports to the interdisciplinary team to facilitate decision making on case plans for the clients.- Develop tools and programs to support reunification work for CYPs and families. Additional Duties for Senior Social Worker role:- Assist in providing professional supervision to team members on practice issues.- Facilitate inter-agency case conferences for suitability of cases and recommendations for reunification.Interested applicants may also send your CV to bernard@cslmch.org.sg
- Degree or Masters in Social Work- At least 2 years (Social Worker) and 4 years (Asst Snr Social Worker) related work experience in children, youth or family work.
- Demonstrates some understanding of reunification work, trauma and attachment theories, and child and adolescent development.- Good communication and interpersonal skills – able to work independently.- Willing to work after office hours .
- A positive attitude with the ability to think out of the box.- Ability to manage stress and crisis effectively while making calm and informed decisions.- Ability to communicate in Malay or Tamil would be a plus.
