About The Role

Requirements:
- Candidate must possess at least Diploma, Advanced, Higher or Graduate Diploma in any field
- Preferably with experience in working with elderly clients in the community setting
- Relevant working experience in healthcare or social service sector
- Applicant must be willing to work 5.5 days per week
- Applicant must be willing to work at Eunos Crescent

Responsibilities:
- Conduct needs assessment survey through outreach and or engagement
- Organise and conduct active ageing programmes to the elderly in the Eunos Crescent community
- Befriend service users through engagement activities and home visits
- Implement new programmes and or services for service users based on needs
- Conduct programme evaluation for all regular programmes and initiate enhancements or improvements
- Coordinate and manage stakeholders communications for programmes and services to service users
- Able to work independently and take ownership in the portfolio assigned
- Any other duties as assigned by Manager


Candidate Requirements
Technical Competencies:

Possess ability to meet targets and deadlines with efficient organisation and time management skills
Possess adaptability to find ways of solving and or pre-empting problems

Core Competencies:

Able to speak in Malay fluently (In order to liaise with the Malay speaking associates)
