About The Role

Job Summary:
• Liaise with internal stakeholders and external referral sources to right-site patients in an expedient manner
• Provide administrative support for the referral screening process
• Conduct social interviews to referrals for suitability for admission

Responsibilities and Duties:

• Attend to enquiries regarding referrals and Hospital admissions
• Assist with administrative tasks related to processing and handling of referrals
• Liaise and coordinate replies with referral sources on outcomes of screening
• Assist in bed management and allocation
• Conduct social interviews to assess viability of care plans and suitability of referrals for admissions and transfers.
• Undertake basic financial counseling at the time of referral screening
• Submit bed statistics to AIC daily

• Assist in the preparation of reports and statistics as required
• Identify areas for improvement or collaboration with internal stakeholders and external referral sources
• Perform other duties as assigned

Candidate Requirements
Technical Competencies:

• Min qualification - Diploma in Social Work or Social Services
• Proficient in Microsoft Office and IT use

Core Competencies:

• Effective communication skills
• Good interpersonal skills

Preferred achievements / characteristics:

• Team worker
• Experience in the healthcare setting (e.g. as a social work assistant or associate) will be an advantage
