Responsibilities:• Conduct needs and intake assessments • Perform case and care planning and goal setting• Implement case and care plans and interventions• Review casework interventions for improvements• Conduct closure of cases upon achieving sustainable service outcomes for clients, families and communities• Maintain and review case notes and documents• Prepare social reports• Conduct environmental scans and community need assessments• Collaborate with other agencies to pool community resources• Implement community development initiatives• Review community development initiatives for improvements• Perform development and planning of programmes to address service gaps• Support evaluation of programmes• Compile and analyse collected data Any other duties as assigned by Supervisor.
• Degree or Post Graduate Diploma in Social Work or related disciplines from recognised universities;
• Required to contribute to the cause of the company and break the stigma related to mental health.• Resilient, passionate to service the community and a team player.
• At least 3 to 5 years of relevant experience. Preferably a registered social worker
