About The Role
Manage cases receiving foster care services and to support programmes or events of Fostering Department and the Agency
Conduct in-depth assessment and prepare Foster Parent Assessment Report (FAR), and present findings at the CIC Approving Panel and follow up on decisions and recommendations Conduct regular CANs assessment to assess how the foster child is doing as well as the foster parents capability in meeting the needs of the child Conduct necessary statutory visits to ensure that the placement is able to safeguard and promote safety, stability and wellbeing Organise and participate in inter-agency efforts to support the welfare and development of the foster child or CYP Provide a 24 hour hotline for foster parents or kinship care per duty schedule and follow up on outcomes Support foster placement to prevent breakdowns in existing care arrangements for the foster child Participate actively in case management Identify at-risk situations and develop strategies and intervention plan to address child safety issues and wellbeing Monitor and evaluate effectiveness of case management and other programme and activities extended to foster or kinship care families Submit accurate operation reports, case reviews, discussions to BT and other governing bodies Requirements Bachelor of Social Work or Graduate Diploma in Social Work or Master of Social Work Registered Social Worker with SASW preferred Experience in working with children or CYP and their families
