• Conduct intake assessments of clients and family/caregivers interviews and register them for the Association’s programmes and services appropriate to their needs• Conduct case management for clients and carry out appropriate clinical interventions, when necessary• Provide counselling and support to clients, their family members and caregivers• Conduct Means-test to assess clients’ eligibility to various assistance schemes and subsidies • Compile and maintain clients’ case history and update case records at all times• Design, implement and facilitate groupwork sessions and workshops for families and caregivers • Coordinate and execute social outreach programmes for the Association• Conduct home visits and able to work occasionally on weekends or evenings• Able to exercise good judgement and discretion in dealing with confidential information
Degree / Graduate Diploma in Social Work or a related discipline with at least 2 – 3 years of relevant experience
