Responsibilities:a. Organize and plan home visits for clients and develop and grow community care services.b. Assess clients’ needs and raise referrals for clients to appropriate social services provider. c. Follow-up with and monitor feedback of clients, to ensure recommended services are in place and aligned with the needs of clients and familiesd. Organize recreational activities for the clients. e. Participate in regular client review discussions with other team members and recommend modification as needed.f. Provide fast response to clients’ needs. g. Ensure timely and accurate reporting to the authority. h. Maintain accurate and timely documentation of client information and ensure relevant agencies are kept updated of changing client needs.i. Manage a team of staff and be a role model.j. Other duties as assigned by management.
a. Degree in healthcare and social services fields. Diploma holder with the right experiences will be consideredb. Be prepared to be available for activities during the non-office hours. c. Team player in a multi-disciplinary team setting with ability to perform basic social screening
