Key Objectives: Assist in the planning and implementation of CREST services. Involve in coordinating outreach events, assist in documentation and indicator tracking to facilitate analysis by the Social worker. Responsibilities: 1. Reach out to collect information and conduct basic-needs assessments of service users. 2. Coordinate, implement and follow up on case plans for service users. 3. Assist in assessing community needs and the compilation of community resources. 4. Liaise with agencies, make service referrals, and assist in developing and implementing programmes. 5. Handle assigned administrative duties.
Ability to engage services users and community partners. Able to use email, calendar, shared drives as well as common software and smart phone apps.
